Then just have 1-2 priest specc reflective shield (better with 2).
Then make them shield the party with reflective and you just jump up and down in the lava while having Sartharion targerted. The lava somehow counts as an attack from Sartharion and you can just keep applying reflective shield to the party while they hog the lava. He will slowly go down and you will get the title.
